NO SHOW/CANCELLATION APPOINTMENT POLICY – EFFECTIVE JUNE 1, 2024

Thank you for entrusting FOI/Florida Sports Orthopaedic & Spine Medicine with your care. When you schedule an appointment, diagnostic test, or procedure, we specifically designate the appropriate amount of time to provide you with the highest quality care. Should you need to cancel or reschedule an appointment, please contact our office as soon as possible and no later than 24 hours before your appointment. Early notification allows our practice to better utilize appointments for other patients in need of prompt medical care. Additionally, if you need to cancel or reschedule your surgery, please contact our office as soon as possible and no later than 72 hours before your surgery.

PLEASE SEE OUR NO SHOW/CANCELLATION FEES FOR THE FOLLOWING SERVICE TYPES:

Office/Therapy Appointments: $25

Diagnostics (includes MRI, CT, EMG, MRA): $100

In-office Procedures: $200

Surgery: $200

Fees are charged to the patient, not the insurance company, and are due at the time of the patient’s next office visit. We understand there may be times when unforeseen emergencies occur, and you may not be able to keep your scheduled appointment. If you should experience extenuating circumstances, please contact our office as soon as possible.

HOW TO CANCEL YOUR APPOINTMENT

You may cancel office appointments by reaching our office Monday-Friday, 8am-5pm. 727-787-5577 or 727-848-4249.
